ABSTRACT

Objective:To investigate the clinical efficacy of neuroendoscopic hematoma removal versus soft channel drainage in the treatment of chronic subdural hematoma. Methods:The clinical data of 102 patients with chronic subdural hematoma who received treatment in Jincheng People's Hospital from May 2018 to May 2020 were retrospectively analyzed. They were divided into the neuroendoscopy group ( n = 50) and the soft channel group ( n = 52) according to different surgical methods. Perioperative indexes, hematoma clearance rate, China Stroke Scale score, the activity of daily living score, and oxidative stress indexes were compared between the two groups. All patients were followed up for 3 months. The incidence of complications during the follow-up period was calculated. Results:The retention time of the drainage tube in the neuroendoscopy group was shorter than that in the soft channel group [(2.45 ± 0.63) days vs. (3.30 ± 0.78) days, t = 6.06, P < 0.001]. The length of hospital stay in the neuroendoscopy group was shorter than that in the soft channel group [(7.14 ± 1.65) days vs. (9.07 ± 2.11) days, t = 5.15, P < 0.001]. The hematoma clearance rate at postoperative 7 days in the neuroendoscopy group was higher than that in the soft channel group [(93.45 ± 5.50)% vs. (81.86 ± 7.24)%, χ2 = 9.12, P < 0.001]. There were no significant differences in operation time and intraoperative blood loss between the two groups (both P > 0.05). At postoperative 30 days, the China Stroke Scale score in the neuroendoscopy group was lower than that in the soft channel group [(12.74 ± 2.23) points vs. (18.67 ± 2.45) points, t = 12.79, P < 0.001]. The activity of daily life score in the neuroendoscopy group was significantly higher than that in the soft channel group [(77.69 ± 7.11) points vs. (91.35 ± 7.25) points, t = 9.60, P < 0.001]. At postoperative 7 days, glutathione peroxidase level in the neuroendoscopy group was significantly lower than that in the soft channel group [(130.75 ± 13.66) U/L vs. (148.60 ± 14.64) U/L, t = 6.37, P < 0.001]. Malondialdehyde level in the neuroendoscopy group was significantly lower than that in the soft channel group [(5.11 ± 0.65) nmol/L vs. (6.19 ± 0.74) nmol/L, t = 7.83, P < 0.001]. Superoxide dismutase level in the neuroendoscopy group was significantly higher than that in the soft channel group [(275.60 ± 22.33) U/L vs. (254.60 ± 18.55) U/L, t = 5.15, P < 0.001]. There was no significant difference in the incidence of complications between the two groups ( P > 0.05). Conclusion:Compared with soft channel drainage, neuroendoscopic hematoma removal can obtain better short-term curative effects and less oxidative stress response in the treatment of chronic subdural hematoma. Neuroendoscopic hematoma removal does not increase the incidence of postoperative complications and is highly safe.

ABSTRACT

Objective:To investigate the diagnostic value of CT-guided puncture biopsy combined with serum gamma-glutamyltransferase (GGT) and abnormal prothrombin (PIVKA-Ⅱ) in serum alpha-fetoprotein(AFP) negative primary liver cancer (PHC).Methods:Eighty patients with AFP negative PHC treatment in Fuyang Women and Children′s Hospital from January 2018 to March 2021 were selected as AFP negative PHC group, and another 85 patients diagnosed with benign liver tumor during the same period were selected as the control group retrospectively. The patients of the two groups underwent CT-guided biopsy and the levels of GGT and PIVKA-Ⅱ were detected. The single diagnostic value and combined diagnostic value of AFP negative PHC were analyzed by receiver operating characteristic (ROC) curve.Results:Seventy-five of the 80 patients in the AFP negative PHC group were confirmed as liver malignant lesions by biopsy, with a coincidence of 93.75%, and 84 of the 85 patients in the control group were confirmed as liver benign lesions by biopsy, with a coincidence of 98.82%. The levels of AFP, GGT and PIVKA-Ⅱ in AFP negative PHC group were significantly higher than those in the control group: (175.67 ± 39.58) μg/L vs. (18.74 ± 7.42) μg/L, (1 245.37 ± 255.41) U/L vs. (486.63 ± 89.05) U/L, (385.49 ± 30.27) AU/L vs. (25.84 ± 7.66) AU/L, there were statistical differences ( P<0.05). Spearman correlation analysis showed that serum AFP was positively correlated with GGT and PIVKA-Ⅱ ( r = 0.858 and 0.429, P<0.05). The results of ROC curve showed that the area under curve of CT-guided biopsy combined with GGT and PIVKA-Ⅱ in the diagnosis of AFP negative PHC was 0.877, the sensitivity was 91.19%, the specificity was 87.34%. Conclusions:CT-guided biopsy combined with GGT and PIVKA-Ⅱ detection of AFP negative PHC can effectively improve the diagnostic value.

ABSTRACT

It is difficult to insert long-term dialysis catheters after severe stenosis or occlusion of the internal jugular vein and innominate vein. We used REcanalisation and balloon-oriented puncture for Re-insertion of dialysis catheter in nonpatent central veins (REBORN) in seven patients with severe central venous lesions, and all patients were inserted with long-term dialysis catheters successfully. None had severe complications such as pneumothorax, hemothorax, or pulmonary embolism during operation. All catheters functioned well after postoperative follow-up of 2 months. REBORN provides a novel approach to establish difficult dialysis pathways.

ABSTRACT

Objective:To investigate the practicality and safety of performing a radical prostatectomy(RP)shortly after the diagnosis of prostate cancer using a combination of prostate targeted biopsy and intraoperative frozen section.Methods:Prospective enrollment was conducted for patients suspected of having prostate cancer based on abnormal prostate specific antigen(PSA)levels.The inclusion criteria for the study were as follows: patients aged 80 years or younger with an ECOG score of 1 or lower.Prior to biopsy, patients underwent both prostate magnetic resonance imaging(MRI)and prostate specific membrane antigen positron emission tomography/computed tomography(PSMA PET/CT)to determine the likelihood of prostate cancer with clinical stages within T 2-3aN 0M 0.In order to be included in the study, patients must agree to receive RP after their prostate cancer diagnosis has been confirmed by biopsy.All enrolled patients underwent a targeted prostate biopsy, consisting of 1-2 cores.These specimens were then examined through frozen section analysis.For patients diagnosed with prostate cancer through intraoperative frozen section pathology, RP was immediately performed.In this study, transperineal prostate targeted+ systematic biopsy was utilized for patients with undiagnosed prostate cancer.Additionally, routine pathological examination of specimens was conducted.The study analyzed the baseline data, surgical conditions, pathological results, and follow-up information of patients in a descriptive manner. Results:Seven patients, ranging in age from 54 to 77 years with a mean age of 66.7 years, were enrolled in the study.Their mean PSA level was 12.668 μg/L, ranging from 4.359 to 22.195 μg/L.Of these patients, 4 had a PI-RADS score of 4 and 3 had a score of 5.The maximum diameter of the index lesion was 1.3 cm, ranging from 0.5 to 2.2 cm.PSMA PET/CT scores were 4 in 1 case and 5 in 6 cases.The index lesions detected by PSMA PET/CT were consistent with those detected by MRI, and the maximum standardized uptake value(SUVmax)was 15.7, ranging from 5.3 to 39.4.Prostate cancer was diagnosed through targeted biopsy and intraoperative frozen section pathology.Four cases had a Gleason score of 3+ 3=6, while one case had a Gleason score of 3+ 4=7, another had a score of 4+ 3=7, and the last had a score of 4+ 4=8.All patients underwent RP treatment immediately after the prostate cancer diagnosis.Only one patient had slight adhesion at the apex of the prostate, while the other six patients were evaluated by surgeons as having no obvious adhesion at the apex.All surgeries were completed successfully, with a mean operation time of 149.7(ranging from 108 to 255)minutes.After RP, whole mount pathology results indicated that all cases were prostate adenocarcinoma, with a Gleason score of 3+ 4=7 in four cases and 4+ 3=7 in three cases.The pathological stages were pT2 in three cases and pT3a in four cases, with five cases having negative surgical margins and two cases with positive surgical margins.During the study, all patients were monitored for a period of 5.4 months(ranging from 3 to 7 months)and no complications of Clavien Dino≥Ⅰ were observed.PSA levels were measured at 6 weeks and 3 months after surgery, with readings of 0.020 μg/L(ranging from 0 to 0.079 μg/L)and 0.016 μg/L(ranging from 0 to 0.087 μg/L), respectively.No hormonal therapy or radiotherapy was administered during this time.Four patients were able to recover from urinary continence.Conclusions:Based on a combination of MRI and PSMA PET/CT, it is both safe and feasible to promptly perform RP following the diagnosis of prostate cancer through targeted biopsy for index lesions, along with intraoperative frozen section.

ABSTRACT

La recurrencia del cáncer diferenciado de tiroides (CDT) a nivel del cuello ocurre en 5%-20% de los casos. El estudio citológico mediante punción de adenopatías ha sido utilizado para confirmar los hallazgos ecográficos sospechosos de malignidad, su sensibilidad varía entre 75%-85%. El objetivo del estudio es evaluar la utilidad de la tiroglobulina (Tg) medida en la punción por aspiración de aguja fina (PAAF) (Tg-PAAF), en el diagnóstico de metástasis ganglionares de pacientes en seguimiento por CDT mayores de 18 años. Se realizó un estudio retrospectivo, descriptivo y observacional en una muestra de 14 pacientes predominantemente de sexo femenino (71,4%), con edad promedio 40,9 ± 2,9 años. Resultados: la variante CDT unifocal fue la más frecuente. De los 22 ganglios sospechosos la mitad tuvieron Tg-PAAF ≥ 1 ng/ml. Todas las adenitis reactivas tuvieron un resultado < 1 ng/ml, en cambio las adenopatías metastásicas obtuvieron un resultado ≥ 1 ng/ml. El 85,7% de pacientes tuvieron anticuerpos anti-Tg ≥10 UI/ml (5 pacientes con valores de Tg-PAAF ≥1 ng/ml y siete pacientes con Tg-PAAF < 1 ng/ml) y 14,3% tuvieron valores < 10 UI/ml (todos con Tg-PAAF <1 ng/ml). Se realizaron cuatro vaciamientos ganglionares, en todas se encontró metástasis de CDT. Conclusiones: la Tg-PAAF es un buen estudio para el diagnóstico de metástasis ganglionares en pacientes en seguimiento de CDT. Dado los resultados obtenidos en este trabajo, apoyado en la búsqueda bibliográfica, el uso de la Tg-PAAF tiene un gran valor diagnóstico para detectar metástasis ganglionares en el seguimiento del CDT por lo que se recomienda su uso junto con la citología y/o la anatomía patológica.


Recurrence of differentiated thyroid cancer (DTC) in the cervix is 5-20% of cases. Fine-needle aspiration cytology of adenopathies has been used to confirm ultrasound findings when suspicions of being malignant. The study aims to evaluate usefulness of fine-needle aspiration of lymph gland to diagnose gland metastases in patients over 18 years old under papillary thyroid cancer follow-up. Retrospective, descriptive and observational study in a 14-patient-sample, mainly female (71,4%), with an average age of 40.9 ± 2.9 years old. Results: single tumor focus papillary thyroid cancer was the most common type of thyroid cancer found. 50% of the 22 suspicious glands had FNATg ≥1ng/ml. All reactive adenitis had measurements < 1ng/ml, whereas metastatic adenopathies results were ≥ 1ng/ml. 85.7% of patients had anti-TG Ac ≥ 1ng/ml (5 patients with FNATg values ≥1ng/ml and 7 patients with FNATg < 1ng/ml), 14.3% of which obtained results < 10 UI/ml (all of them with FN1 ng/ml). Gland emptying was performed in 4 cases, and papillary thyroid cancer metastases was found in all of them. Conclusions: FNATg is a good study to diagnose gland metastases in patients under differentiated thyroid cancer follow-up. Given the results of this study, supported by a bibliographic search, the use of FNATg has a great diagnostic value to detect gland metastases in the follow up of differentiated thyroid cancer, and thus it is recommended along with cytological and/or pathology studies.


A recorrência do câncer diferenciado de tiroides (CDT) no nível do pescoço, ocorre em 5-20% dos casos. O estudo citológico mediante punção de linfadenopatia foi utilizado para confirmar os achados ecográficos suspeitos de malignidade; sua sensibilidade varia entre 75-85%. O objetivo do estudo era avaliar a utilização da tiroglobulina (Tg) medida na punção por aspiração por agulha fina (PAAF) (Tg-PAAF), no diagnóstico de metástase ganglionar de pacientes em seguimento por CDT maiores de 18 anos. Realizou-se um estudo retrospectivo, descritivo e observacional em uma amostra de 14 pacientes predominantemente de sexo feminino (71,4%), com idade média de 40,9 ± 2,9 anos. Resultados: a variante CPT unifocal foi a mais frequente. Dos 22 linfonodos suspeitos, a metade apresentou Tg-PAAF ≥ 1ng/ml. Todas as adenites reativas apresentaram um resultado < 1ng/ml, no entanto as linfadenopatias metastásicas tiveram um resultado ≥ 1ng/ml. 85,7% dos pacientes apresentam Ac anti-Tg ≥10 UI/ml (5 pacientes com valores de Tg-PAAF ≥1ng/ml e 7 pacientes com Tg-PAAF < 1ng/ml) e 14,3% valores < 10 UI/ml (todos com Tg-PAAF <1 ng/ml). Foram feitos 4 esvaziamentos ganglionares, em todos foram encontradas metástases da CPT. Conclusões: o Tg-PAAF é um bom método para o diagnóstico de metástase ganglionar em pacientes em seguimento de CDT. Considerando os resultados obtidos neste trabalho, apoiado na bibliografia, o uso do Tg-PAAF tem um grande valor diagnóstico para detectar metástase ganglionar no seguimento do CDT; por essa razão recomenda-se seu uso junto com a citologia e/ou anatomia patológica.

Abstract Introduction: There are several approaches for pericardiocentesis. However, there is no definite suggestion about puncture location after cardiac surgery. The purpose of this study is to examine whether there is any difference regarding puncture location during pericardiocentesis in postoperative cardiac tamponade comparing to nonsurgical cardiac tamponade. Methods: We retrospectively analyzed patients who had undergone pericardiocentesis from August 2011 to December 2019. Patients were examined in two groups, nonsurgical and postsurgical, based on the etiology of pericardial tamponade. Clinical profiles, echocardiographic findings, and procedural outcomes were identified and compared. Results: Sixty-eight pericardiocenteses were performed in this period. The etiology of pericardial effusion was cardiac surgery in 27 cases and nonsurgical medical conditions in 41 cases. Baseline demographic variables were similar between the surgical and nonsurgical groups. Loculated effusion was more common in the postsurgical group (48.1% vs. 4.9%, P<0.001). Maximal fluid locations were different between the groups; right ventricular location was more common in the nonsurgical group (36.6% vs. 11.1%, P=0.02), while lateral location was more common in the postsurgical group (12.2% vs. 40.7%, P=0.007). Apical drainage was more frequently performed in the postsurgical group compared to the nonsurgical group (77.8% vs. 53.7%, P=0.044). Conclusion: Apical approach as a puncture location can be used more frequently than subxiphoid approach for effusions occurred after cardiac surgery compared to nonsurgical effusions. Procedural success is prominent in this group and can be the first choice of treatment.

ABSTRACT

Objective:To explore the effect of minimally invasive hematoma puncture and drainage in the treatment of elderly patients with cerebral hemorrhage by using 3D slicer and Sina software to conduct 3D reconstruction and preoperative localization of intracerebral hematoma.Methods:A total of 74 elderly patients with a first-onset intracerebral hematoma aged ≥75 years, having surgical indications and stable vital signs were grouped into 3D slicer plus Sina software localization group(as group A, n=40)or CT localization group(as group B, n=34). Based on the localization, hematoma puncture and drainage were performed after local anesthesia.Preoperative preparation time, hematoma location, puncture success rate, postoperative hematoma clearance rate, postoperative re-bleeding rate and GCS score were statistically analyzed.Glasgow coma scale(GCS)scores were used in predicting the mortality.Results:The preoperative preparation time was significantly shorter in group A than in group B[(5.5±3.4)min vs.(8.5±2.7)min, t=3.337, P=0.001]. The success rate of hematoma puncture and drainage(90.0% and 70.6%, χ2=4.51, P=0.034)and postoperative hematoma clearance rate[(87.5±3.4)% and(80.3±2.7)%, t=10.10, P=0.000]were higher in group A than in group B. There were no significant differences in operative time, the accuracy of hematoma localization, re-bleeding rate and GCS score between the two groups( P>0.05). Conclusions:3D slicer plus Sina software can precisely locate the intracerebral hematoma, and minimally invasive hematoma puncture and drainage of intracerebral hematoma under local anesthesia were safe and effective in the treatment of elderly patients with intracerebral hemorrhage.

ABSTRACT

Objective:To compare the clinical efficacy of two-dimensional (2D) fluoroscopic guided robot-assisted and conventional TESSYS surgery in the treatment of lumbar disc herniation, the feasibility and precautions of 2D fluoroscopic guided robot-assisted TESSYS surgery were discussed.Methods:A retrospective analysis was performed on 34 male and 36 female patients aged 44.57±6.10 years (range 27 to 60 years) who received 2D guided robot-assisted TESSYS surgery and conventional TESSYS surgery from July 2019 to April 2020. 2D guided robot assisted TESSYS surgery group (the robot group) 32 cases, conventional TESSYS surgery (the conventional group) 38 cases. The number of puncture, number of fluoroscopy, puncture-channel time, operation time, visual analogue scale (VAS) and Oswestry disability index (ODI) before and after surgery and at the last follow-up were compared between the two groups to verify the clinical efficacy, and the complications were analyzed.Results:There were no significant differences in age, gender, body mass index, surgical level, Michigan State University (MSU) grading and average follow-up time between the robot group and the conventional group (all P>0.05). All patients completed surgery. The number of puncture times was 1.22±0.79 in the robot group and 4.66±1.86 in the conventional group, and the difference was statistically significant ( t=10.320, P<0.001). The number of fluoroscopy in the robot group was 10.97±1.96 times, and that in the conventional group was 17.45±4.30 times, the difference was statistically significant ( t=8.313, P<0.001). The puncture-channel time of the robot group was 10.66±3.62 min and that of the conventional group was 20.21±5.47 min, and the difference was statistically significant ( t=8.446, P<0.001). The operation time of the robot group was 62.25±6.68 min, and that of the conventional group was 72.89±10.48 min, the difference was statistically significant ( t=4.956, P<0.001). VAS scores of the robot group were 6.91±0.93 points before surgery, 2.97±0.65 points 1 d after surgery, and 1.53±0.51 points at the last follow-up, while those of the conventional group were 7.29±1.14 points before surgery, 2.89±0.56 points 1 d after surgery, and 1.42±0.50 points at the last follow-up. The ODI index of the robot group was 40.13%±1.54% before surgery, 17.28%±1.69% 1 day after surgery, and 10.84%±1.25% at the last follow-up, while that of the conventional group was 40.03%±1.46% before surgery, 17.42%±2.45% 1 day after surgery, and 10.92%±1.17% at the last follow-up. There were no statistically significant differences (all P>0.05). Two patients (6.3%) in the robot group and four (10.5%) in the conventional group had residual disc, there was no significant difference ( P>0.05). Conclusion:2D guided robot assisted TESSYS surgical treatment of lumbar disc herniation, which can plan puncture path in the stage of percutaneous target puncture, rigid guidance and accurate puncture target, and has obvious advantages in reducing puncture times and fluoroscopy times.

Objective:To explore the diagnosis, treatment and prevention of biliary hemorrhage after percutaneous transhepatic choledochoscopic lithotomy(PTCSL).Methods:Retrospective summary and analysis were made on 48 cases of PTCSL completed by the hepatobiliary surgery department of Hunan Provincial People′s Hospital (the First Affiliated Hospital of Hunan Normal University) from January 2016 to December 2020.Results:Biliary hemorrhage occurred in 3 cases after operation, of which 1 case was considered to have a small amount of biliary mucosa bleeding, and the bleeding was stopped after blood transfusion, hemostatic agents and appropriate fluid rehydration. In the other case, a small amount of blood oozing around the T-tube sinus canal was accompanied by a small amount of bloody fluid in the T-tube, which was considered to cause arterioles or venules accompanied by bile duct bleeding during sinus expansion during operation, and the bleeding stopped after compression. Another patient underwent a short time drainage of hemorrhagic fluid ≥100 ml in T tube 15 days after operation, and was prepared for interventional therapy while receiving blood transfusion products and conservative treatment. Celiac arteriography was performed, and pseudoaneurysm was found in the right anterior lobe of the liver. Embolization was performed later and the patient discharged 10 days after embolization.Conclusions:Biliary hemorrhage is one of the more common complications after PTCSL. Increasing the understanding of this complication provides a guarantee for the safety of surgery and has clinical significance for patients with accelerated postoperative recovery.

ABSTRACT

Objective:To investigate the clinical efficacy of 3D printed guide plate-assisted hematoma puncture and catheter drainage in the treatment of a moderate volume of hypertensive intracerebral hemorrhage in the basal ganglia.Methods:The clinical data of 42 patients with a moderate volume of hypertensive intracerebral hemorrhage in the basal ganglia who received treatment with 3D printed guide plate-assisted hematoma puncture and catheter drainage in Dazhu Hospital of North Sichuan Medical College from January 2016 to January 2021 were retrospectively analyzed. In the traditional puncture group, there were 19 patients who received traditional punctures according to preoperative cranial CT findings (traditional puncture group). In the 3D printed guide plate puncture group, there were 23 patients who received hematoma puncture and catheter drainage assisted by a 3D printed guide plate. Preoperative preparation time (from admission to operation), operative time, the number of hematoma punctures, hematoma clearance rate, postoperative indwelling time of drainage tube, residual hematoma rate, the occurrence of puncture canal bleeding, intracranial infection, Glasgow Coma Scale score measured at 3 and 7 days after surgery, and Glasgow Outcome Scale score measured at 1, 3 and 6 months after surgery were compared between the two groups.Results:There were no significant differences in preoperative preparation time and operative time between the two groups ( t = 0.25, 0.40, both P > 0.05). 3D-printed guide plate-assisted hematoma puncture reduced the probability of reveiving one more hematoma puncture during the surgery. There was no significant difference in the number of hematoma puncture between the two groups ( χ2= 0.48, P > 0.05). There were no significant differences in the probabilities of puncture canal bleeding and intracranial infection between the two groups ( χ2 = 0.05, 0.03, both P > 0.05). Postoperative indwelling time of the drainage tube in the 3D printed guide plate puncture group was significantly shorter than that in the traditional puncture group [(3.10 ± 0.38) vs. (3.46 ± 0.52) days, t = 2.65, P < 0.05]. The residual hematoma rate in the 3D printed guide plate puncture group was significantly lower than that in the traditional puncture group [(32.04 ± 5.33)% vs. (37.37 ± 5.51)%, χ2 = 3.20, P < 0.05]. There were significant differences in Glasgow Coma Scale score measured at 3 and 7 days after surgery between the two groups [(12.04 ± 1.19) points vs. (11.26 ± 0.93) points, (13.65 ± 0.88) points vs. (12.94 ± 0.97) points, t = 2.33, 2.46, both P < 0.05]. Glasgow Outcome Scale score measured at 1, 3, and 6 months after surgery in the 3D printed guide plate puncture group was 18, 21, and 22 points, respectively, which was significantly higher than 9, 11, and 12 points in the traditional puncture group ( χ2 = 4.34, 4.69, 5.17, all P < 0.05). Conclusion:3D printed guide plate assisted hematoma puncture and catheter drainage for treatment of a moderate volume of hypertensive intracerebral hemorrhage in the basal ganglia can increase the accuracy of puncture, decrease hematoma residual rate, improve short- and long-term prognosis, does not prolong preoperative preparation time. Moreover, it costs low, is safe, and is easy to learn.

ABSTRACT

Objective:To investigate the efficacy of different surgical treatments for hypertensive cerebral hemorrhage in older adult patients and their effects on traumatic stress and cerebral edema.Methods:A total of 100 older adult patients with hypertensive cerebral hemorrhage who received treatment in Zhejiang Xin'an International Hospital from January 2018 to June 2020 were included in this study. They underwent either craniotomy (craniotomy group, n = 50) or hard channel minimally invasive puncture drainage (minimally invasive puncture group, n = 50) according to the willingness of patients and their close relatives. Perioperative indexes, Barthel index after treatment, nerve injury indexes before and after treatment, prognosis related indexes, trauma stress indexes and brain edema were compared between the two groups. Results:Operative time, intraoperative blood loss and postoperative hospital stay in the craniotomy group were (147.21 ± 31.35) minutes, (289.74 ± 22.75) mL and (42.74 ± 6.82 ) days, respectively, which were significantly longer or greater than (41.88 ± 7.19) minutes, (4.62 ± 0.88) mL and (16.27 ± 4.02) days in the minimally invasive puncture group ( t = 38.73, 62.17, 23.17, all P < 0.001). Barthel index at 1 and 3 months after treatment in the minimally invasive puncture group was (63.11± 9.64) and (93.51 ± 11.38), respectively, which was significantly greater than (44.78 ± 8.85) and (81.29 ± 10.37) in the craniotomy group ( t = 3.17, 6.21, both P < 0.05). Before treatment, there were no significant differences in nerve injury index, prognosis index, trauma stress index and brain edema between the two groups (all P > 0.05). At different time points after treatment, each indicator in the minimally invasive puncture group was significantly superior to that in the craniotomy group (all P < 0.05). Conclusion:Hard channel minimally invasive puncture drainage exhibits advantages over traditional craniotomy in the treatment of hypertensive cerebral hemorrhage in older adult patients. Hard channel minimally invasive puncture drainage can more greatly reduce injury to brain tissue, better control nerve injury and brain edema, and more remarkably improve patient's quality of life than traditional craniotomy.

ABSTRACT

Objective:To investigate the application effect of arteriovenous fistula cannulation by buttonhole technique with blunt needles in patients subjected to hemodialysis.Methods:Seventy-six patients who underwent hemodialysis in Hemodialysis Room, The First Hospital of Jiaxing between June 2016 and June 2019 were included in this study. They were randomly divided into control and observation groups ( n = 38/group). The control group was subjected to arteriovenous fistula cannulation using a regional puncture method. The observation group was subjected to arteriovenous fistula cannulation by buttonhole technique with blunt needles. One-time success rate of puncture was recorded in each group. The maximum transverse diameters of the fistula before and after 6 months of puncture were measured. Severity of pain at the time of puncture was evaluated using Visual Analogue Scale (VAS). Complications were recorded in each group. Patient compliance was investigated using a questionnaire. Patient's quality of life was evaluated using Kidney Disease Quality of Life Short Form 1.3. Results:One-time success rate of puncture in the observation group was significantly higher than that in the control group [97.37% (37/38) vs. 84.21% (32/38), χ2 = 3.93, P = 0.04]. The maximum transverse diameter of the fistula in the observation group was significantly smaller than that in the control group [(4.36 ± 0.11) mm vs. (7.26 ± 0.48) mm, t = 36.30, P < 0.01]. At 6 months after puncture, the maximum transverse diameter of the fistula in each group increased compared with that before puncture (both P < 0.05). The VAS score in the observation group was significantly lower than that in the control group [(0.82 ± 0.24) points vs. (3.11 ± 0.32) points, t = 35.29, P < 0.01]. The incidence of complications in the observation group was significantly lower than that in the control group [5.26% (2/38) vs. 21.05% (8/38), χ2 = 4.15, P = 0.04]. The compliance score in the observation group was significantly higher than that in the control group [(36.32 ± 3.21) points vs. (27.18 ± 2.69) points, t = 13.45, P < 0.01]. The scores of role limitations caused by physical health problems, pain, role limitations caused by emotional health problems in the observation group were significantly lower than those in the control group (all P < 0.05). The scores of physical functioning, general health perceptions, emotional well-being, social functioning, energy/fatigue, and overall health rating item in the observation group were significantly higher than those in the control group (all P < 0.05). Conclusion:Arteriovenous fistula cannulation by buttonhole technique with blunt needles can increase success rate of puncture, reduce pain, decrease the incidence of complications and exhibit protective effects on arteriovenous fistula, thereby improving patient's quality of life and increasing treatment compliance.

ABSTRACT

Objective:To investigate the value of prostate biopsy guided by transrectal real-time ultrasonic elastography (TRTE) combined with peak strain index (PSI) in the diagnosis of prostate cancer and the correlation with TRTE score and pathological Gleason score.Methods:A total of 80 patients with suspected prostate cancer who underwent TRTE in the First Affiliated Hospital of Hebei North University from January 2019 to December 2019 were selected. The PSI for suspicious lesions was measured, and targeted puncture biopsy guided by TRTE combined with PSI was performed on the patients, and then followed by systematic puncture biopsy. The outcomes of targeted biopsy and systematic biopsy were analyzed. Taking pathological biopsy results as the gold standard, the detection rates of prostate cancer and benign prostate lesions detected by both biopsies methods were compared; the prostate volume, serum prostate specific antigen (PSA) level and PSI were compared between patients with prostate cancer and benign prostatic lesions. The receiver operating characteristic (ROC) curve and the area under the curve (AUC) were used to determine the best cut-off value of PSI in the diagnosis of prostate cancer. The values of conventional ultrasound versus TRTE combined with PSI in the diagnosis of prostate cancer were assessed. The positive rate of biopsy puncture points under the guidance of TRTE combined with PSI was compared with that of systematic biopsy. The correlation between TRTE score and pathological Gleason score of prostate malignant lesions was analyzed.Results:Among 80 patients, 45 patients (56.25%) were diagnosed as prostate cancer by prostate puncture biopsy, and 35 patients (43.75%) were benign prostate lesions. Among 45 patients with prostate cancer, 42 cases (93.33%) of prostate cancer were detected by using TRTE combined with PSI-guided targeted puncture biopsy, and 38 cases (84.44%) of prostate cancer were detected by using systematic puncture biopsy; there was no significant difference in the detection rate of prostate cancer by both biopsies methods ( χ2 = 1.80, P = 0.180). The level of serum PSA and PSI value in the prostate cancer group were higher than those in the benign prostate lesion group, and the difference was statistically significant ( t value was 65.28 and 14.93, all P < 0.05). The clinical value of PSI value in the diagnosis of prostate cancer was analyzed by using ROC curve. The results showed that the AUC was 0.857 (95% CI 0.772-0.941), and the optimal cut-off value of PSI was 5.68; PSI ≥ 5.68 was treated as the malignant cancer and PSI < 5.68 was treated as the benign cancer. The sensitivity, specificity and accuracy of TRTE combined with PSI in the diagnosis of prostate cancer were 91.11%, 94.29%, and 92.50%, respectively, which were higher than those of conventional ultrasound (73.33%, 68.57% and 71.25%), and the differences were statistically significant (all P < 0.05). A total of 89 suspected lesions were detected in 80 patients through TRTE combined with PSI, and each suspected lesion was detected by using 2-needle targeted puncture biopsy. There were 178 needles in total including 88 needles of prostate cancer and the positive rate of puncture points was 49.44% (88/178); there were 800 needles in total detected by using 10-needle systematic puncture biopsy including 203 needles of prostate cancer and the positive rate of puncture points was 25.38% (203/800); the positive rate of puncture points guided by TRTE combined with PSI puncture biopsy was higher than that by systematic puncture biopsy, and the difference was statistically significant ( χ2 = 40.337, P < 0.05). For prostate malignant lesions, the Spearman correlation analysis showed that TRTE score was positively correlated with pathological Gleason score ( r = 0.618, P < 0.05). Conclusion:TRTE combined with PSI-guided targeted puncture biopsy plays an important role in the diagnosis of prostate cancer, and it can effectively improve the positive rate of puncture points.

Objective:To evaluate delayed laparoscopic cholecystectomy (DLC) after percutaneous transhepatic gallbladder drainage (PTGBD) in acute cholecystitis.Methods:Clinical data of 64 patients who were diagnosed moderate (grade Ⅱ) acute cholecystitis by the 2018 Tokyo Guidelines in acute phase and underwent delayed LC at our hospital from Jan 2018 to Jan 2021 were compared between two groups ie PTGBD treatment (21 cases)in acute stage before DLC and DLC without PTGBD group (43 cases). The difficulty score of TG18 was used to evaluated every surgical procedure of the cases by reviewing the operation videos.Results:Patients in DLC after PTGBD group had a longer hospital stay and operation time, more blood lose and higher difficulty score than the DLC without PTGBD group(all P<0.05). There was no statistically significant difference in the conversion rate and morbidity rate between the two groups( P>0.05). Conclusion:This study fails to show there is any if ever benefit of PTGBD before DLC over DLC without PTGBD in the management of Grade Ⅱ acute cholecystitis.

Objective:To compare the effects of ultrasound-guided dynamic needle tip positioning (DNTP) and long axis in-plane (LAX-IP) techniques for axillary vein puncture and catheterization.Methods:One hundred Society of Anesthesiologists physical statusⅠ-Ⅲ patients of both sexes, aged 18-64 yr, with body mass index of 20-28 kg/m 2, scheduled for elective axillary vein cannulation, were divided into 2 groups ( n=49 each) using the random number table method: DNTP group and LAX-IP group.Axillary vein puncture was performed using DNTP technique and LAX-IP technique under ultrasound guidance in DNTP group and LAX-IP group, respectively.Successful puncture at first attempt, overall successful catheterization, the number of needle tip redirection, and axillary vein puncture time and catheterization time were recorded.The occurrence of complications such as axillary artery puncture, posterior wall penetration of axillary vein, hematoma formation, pneumothorax, and nerve injury was recorded. Results:Compared with group LAX-IP, the success rate of puncture at first attempt was significantly increased, the number of cases required needle redirection was decreased, and the puncture time was shortened ( P<0.05), and no significant change was found in the logarithm of the posterior wall penetration of axillary vein in group DNTP ( P>0.05). No complications such as arterial puncture, hematoma, pneumothorax, or nerve injury occurred in two groups. Conclusions:Compared with LAX-IP technique, ultrasound-guided DNTP technique can dynamically observe the position of the needle tip, the operation is simple and safe, and it is worthy of clinical promotion when used for axillary vein puncture and cannulation.

ABSTRACT Objective To present a study protocol to compare glucose and reflexology in pain relief in neonate intensive care during arterial punctures. Methods A randomized, controlled, double-blind clinical trial protocol will be carried out at a teaching hospital maternity, with30 newborns admitted to neonate intensive care who are to undergo blood collection by arterial puncture. They will be randomly assigned to a control group (25% glucose) or an intervention group (foot reflexology). The primary outcome will be neonate pain scores during and after arterial puncture. Secondary outcomes will be crying time and variation in neonates' vital signs during and after the arterial puncture procedure. Registration number RBR-639bff. Discussion The results of this trial will provide new insights into the most appropriate intervention for the relief of neonate pain during painful procedures.


RESUMEN Objetivo Presentar un protocolo de estudio para comparar glucosa y reflexología en el alivio del dolor en cuidados intensivos neonatales durante la punción arterial. Método Protocolo de ensayo clínico aleatorizado, controlado, doble ciego, en 30 recién nacidos en cuidados intensivos neonatales en la maternidad de un hospital escuela, que necesiten extracción de sangre por punción arterial. Serán asignados aleatoriamente a un grupo control (25% de glucosa) o a un grupo de intervención (reflexología podal). El resultado primario serán los escores de dolor neonatal durante y después de la punción arterial. Los resultados secundarios serán el tiempo de llanto y la variación en los signos vitales de los recién nacidos durante y después del procedimiento de punción arterial. Número de registro RBR-639bff. Discusión Los resultados de este ensayo proporcionarán nuevos conocimientos sobre la intervención más adecuada para el alivio del dolor neonatal durante los procedimientos dolorosos.

Serão distribuídos aleatoriamente em grupo controle (glicose 25%) ou grupo intervenção (reflexoterapia podal). O desfecho primário será escores de dor neonatal durante e após a punção arterial. Os desfechos secundários serão o tempo de choro e variação nos sinais vitais dos neonatos durante e após o procedimento da punção arterial. Número do registro RBR-639bff. Discussão Os resultados deste ensaio fornecerão novos conhecimentos sobre a intervenção mais adequada para o alívio da dor neonatal durante procedimentos dolorosos.

ABSTRACT Objective To analyze in the scientific literature the effects of massage on pain relief in newborns submitted to puncture. Method Systematic review with meta-analysis performed in October 2020, using PubMed, Web of Science, CINAHL, Scopus, Cochrane and Gale databases. Studies without time frame were included, which used massage as the main technique for relieving neonatal pain during puncture. Data were extracted using standardized forms and the synthesis of results occurred in a descriptive way. Results From the 12 studies included, massage was effective in pain relief in 83.3% of the studies. The comparative meta-analysis of massage versus routine care that assessed duration of crying obtained a statistically significant result (p = 0.0002; 95% CI -85.51 to -27.09). Conclusion Massage contributes to neonatal pain relief by reducing pain score and reducing crying time in newborns submitted to puncture.


RESUMEN Objetivo Analizar en la literatura científica los efectos del masaje en el alivio del dolor en recién nacidos sometidos a punción. Se incluyeron estudios sin corte temporal, que utilizaron el masaje como técnica principal para el alivio del dolor neonatal durante la punción. Resultados De los 12 estudios incluidos, el masaje fue efectivo para aliviar el dolor en el 83,3% de los estudios. RESUMO Objetivo: Avaliar a intensidade de dor durante a punção arterial realizada em recém-nascidos internados em uma unidade de cuidados progressivos neonatais e avaliar a percepção do profissional em relação à dor neonatal. Métodos: Estudo observacional analítico, em que foram observadas 62 punções arteriais realizadas em 35 neonatos. Avaliou-se a dor durante a coleta pela escala Premature Infant Pain Profile. Os profissionais responsáveis pela coleta avaliaram a dor pela escala numérica verbal de zero a dez. Os dados foram submetidos à análise estatística descritiva por meio do programa Statistical Package for the Social Science. Resultados: Entre os recém-nascidos, 30,6% (n = 19) não tiveram dor ou tiveram dor leve (0 - 6), 24,2% (n = 15) apresentaram dor leve a moderada (7 - 11) e 45,2% (28) dor intensa (12 - 21). Constatou-se que os profissionais identificam a dor durante o procedimento. Conclusão: A punção arterial é considerada um procedimento doloroso e pode resultar em dor leve a intensa, sendo necessária a adoção de estratégias sistematizadas de avaliação, possibilitando a intervenção terapêutica adequada.


ABSTRACT Objective: To evaluate pain intensity during arterial puncture performed in newborns admitted to a neonatal progressive care unit and to evaluate the perception of health professionals regarding neonatal pain. Methods: This was an observational analytical study in which 62 arterial punctures were performed in 35 neonates. Pain was assessed during collection using the Premature Infant Pain Profile scale. The health professionals responsible for collection evaluated pain using a verbal numerical scale ranging from zero to ten. The data were subjected to descriptive statistical analysis using the Statistical Package for the Social Science software. Results: Among the newborns, 30.6% (n = 19) had no pain or mild pain (0 - 6), 24.2% (n = 15) had mild to moderate pain (7 - 11) and 45.2% (28) had severe pain (12 - 21). It was found that health professionals identified pain during the procedure. Conclusion: Arterial puncture is considered a painful procedure that can result in mild to severe pain. The adoption of systematic evaluation strategies is necessary to enable appropriate therapeutic intervention.

ABSTRACT BACKGROUND AND OBJECTIVES: Pain and stress are the main factors stimulating behavioral, physiological, and hormonal changes in preterm newborns (PTNB) admitted to a Neonatal Intensive Care Unit (NICU); therefore, the systematic multidimensional assessment of pain at the bedside becomes an essential tool for reducing the impact of brain stimulation, as it guides the appropriate treatment. The objectives of this study were to assess the pain at the bedside of PTNB submitted to peripheral venipuncture or diaper change and to describe the measures for pain relief. METHODS: Longitudinal study, with a prospective cohort of 25 PTNB, carried out in an NICU in the city of Rio de Janeiro. Pain assessment at the bedside was performed at three different times, using the Neonatal Infant Pain Scale (NIPS) and Premature Infant Pain Profile (PIPP), when subjected to peripheral venipuncture for infusion therapy and diaper changes. The scores of the scales were analyzed using the Chi-square test. RESULTS: In the puncture group, there was pain and increased intensity. In the diaper group, three PTNB showed pain at the third assessment. During peripheral venipuncture, the non-pharmacological measures were used: oral 25% glucose, non-nutritive suction and a combination of the two. CONCLUSION: Assessing pain at the bedside using scales helps nursing professionals in pain prevention and treatment, avoiding altered responses of newborns in the face of procedures.


RESUMO JUSTIFICATIVA E OBJETIVOS: A dor e o estresse são os principais fatores de estímulo às alterações comportamentais, fisiológicas e hormonais dos recém-nascidos pré-termos (RNPT) internados na Unidade de Terapia Intensiva Neonatal (UTIN); portanto, a avaliação multidimensional sistemática da dor se torna uma ferramenta imprescindível para a redução do impacto da estimulação cerebral, visto que direciona para o adequado tratamento. Os objetivos deste estudo foram avaliar a dor à beira do leito de RNPT submetidos à punção venosa periférica ou à troca de fraldas e descrever as medidas para o alívio da dor. MÉTODOS: Estudo longitudinal, com uma coorte prospectiva de 25 RNPT, realizado em uma UTIN no município do Rio de Janeiro. Foi realizada avaliação da dor à beira do leito em três momentos distintos, com a Neonatal Infant Pain Scale e Premature Infant Pain Profile, quando submetidos à punção venosa periférica para terapia infusional e troca de fraldas. As pontuações das escalas foram analisadas por meio do teste Qui-quadrado. RESULTADOS: No grupo punção, houve presença de dor e aumento da intensidade. No grupo fralda, três RNPT mostraram presença de dor na terceira avaliação. Durante a punção venosa periférica, foram utilizadas as medidas não farmacológicas; glicose oral a 25%, sucção não nutritiva e a combinação das duas. CONCLUSÃO: A avaliação da dor à beira do leito por meio de escalas auxilia os profissionais de enfermagem na prevenção e tratamento da dor, evitando respostas alteradas do recém-nascido diante de procedimentos.

Objective:To study the efficacy and safety of early abdominal puncture drainage (APD) in severe acute pancreatitis (SAP).Methods:A retrospective study was conducted on 189 patients with SAP who were managed at the Department of Intensive Medicine of the Second Affiliated Hospital of Anhui Medical University from January 2013 to May 2020. According to whether ultrasound-guided APD was performed within one week after admission to ICU, these patients were divided into 2 groups: patients treated with APD (the APD group) and patients treated without APD (the non-APD group). Clinical data, including the acute physiological and chronic health status (APACHE) Ⅱ score, modified Marshall score, sequential organ failure evaluation (SOFA) score, and prognostic indicators including the retroperitoneal percutaneous drainage (PCD) rate and length of hospital stay, were compared between the two groups before and 1 week after surgery.Results:Of the 189 SAP patients in this study, there were 110 males and 79 females, aged (52.5±17.4) years old. On admission to ICU, the blood amylase, C-reactive protein, procalcalonin, interleukin-6, APACHE II score, modified Marshall score and SOFA score in the APD group were significantly higher than those in the non-APD group. After 1 week of treatment, most clinical indicators in the 2 groups were significantly improved, and there were no significant differences between these indicators (all P>0.05). There were no significant differences in the abdominal infection, retroperitoneal PCD and mortality rates between the APD group and the non-APD group ( P>0.05). The length of hospital stay [29 (18, 45) vs 21 (15, 32) d] and ICU stay [5 (3, 11) vs. 7 (5, 17) d] in the APD group were significantly higher than those in the non-APD group ( P<0.05). Conclusion:For patients with SAP with peritoneal effusion, early APD effectively improved the condition and prognosis without increasing the peritoneal infection and mortality rates.
